After evaluation by a physician, they provide a plan of care for each patient based on diagnoses. 


The following criteria generally apply. 


  • If you have difficulty leaving the home
  • If you are calling or making frequent trips to a physicians office
  • If you have been discharged from the hospital
  • If you have wounds not healing or are frequently infected
  • If you need blood work done regularly
  • If you are an unstable diabetic
  • If you have had recent medication changes
  • If you have blood pressure fluctuation
  • If you are leaving a skilled nursing facility within a nursing home, Medicare will pay for continued therapy in your home
